PUBLIC NOTICE REGARDING
DESTRUCTION OF SPECIAL
EDUCATION RECORDS
Special education records which have been collected by the Alexandria Public School related to identification, evaluation, educational placement, or the provision of Special Education in the district, must be maintained under state and federal laws for a period of six (6) years after Special Education services have ended for the student. Special education services end when the student no longer is eligible for services, graduates, completes his or her educational program at age 22, or moves from the district.
This notice is to inform parents/guardians and former students of the Alexandria Public School’s intent to destroy the Special Education Records of students who received Special Education services prior to and during the 2005-2006 school year. Records to be destroyed are as follows:
Individualized Education Programs (IEPs)
Evaluation Reports
Test Protocols
Notifications of Meetings
Notices of Action
Review of Existing Data Summaries
All other personally identifiable information within the Special Education file*
After six (6) years, the records are no longer useful to the district, but they may be useful to the parent/guardian or former student in applying for Social Security benefits, rehabilitation services, college entrance, etc. The parent/guardian or eligible (adult) student, who has reached 18 years of age, may pick up the records in person by December 14 at the following address:

If no student, parent or guardian responds to this public notice, the school district will assume consent to destroy the entire record specific to the student beginning January 1, 2013.
*The district may maintain a permanent record, without time limitation, of a student’s name, address and phone number, his/her grades, attendance record, classes attended, grade level completed and year completed.
