Medica Foundation awards grant to Knute Nelson Foundation

The Medica Foundation awarded the Knute Nelson Foundation a grant to help establish a chronic disease management clinical care program at Knute Nelson to ultimately decrease the emergency room use and re-hospitalizations.

The Medica Foundation awarded the Knute Nelson Foundation a grant to help establish a chronic disease management clinical care program at Knute Nelson to ultimately decrease the emergency room use and re-hospitalizations.

The grant will help create a systematic and patient-centered approach to managing chronic diseases focusing on effective care transitions with outcomes to support successful daily living. It will also foster greater communication among patients, families, staff and other clinical partners.

Centers for Medicare and Medicaid Service report that at least one-third of all ER visits are “avoidable,” meaning non-urgent or ambulatory care sensitive. More than 34 percent of Medicare re-hospitalizations are from individuals with multiple chronic diseases.
