PUBLIC NOTICE REGARDING

DESTRUCTION OF SPECIAL

EDUCATION RECORDS

Special education records which

have been collected by the Alexandria

Public School related to the identifica-

tion, evaluation, educational place-

ment, or the provision of Special Edu-

cation in the district, must be main-

tained under state and federal laws for

a period of six (6) years after Special

Education services have ended for the

student. Special education services

end when the student no longer is eli-

gible for services, graduates, com-

pletes his or her educational program

at age 22, or moves from the district.

This notice is to inform par-

ents/guardians and former students of

Alexandria Public School's intent to

destroy the Special Education records

of students who received Special Edu-

cation services prior to and during the

2003-2004 school year. Records to be

destroyed are as follows:

Individualized Education Programs

(IEPs)

Evaluation Reports

Test Protocols

Notifications of Meetings

Notices of Action

Review of Existing Data Summar-

ies

All other personally identifiable in-

formation within the Special Education

file*

After six (6) years, the records are

no longer useful to the district, but

they may be useful to the parent/guar-

dian or former student in applying for

Social Security benefits, rehabilitation

services, college entrance, etc. The

parent/guardian or eligible (adult) stu-

dent who has reached 18 years of

age, may pick up the records in per-

son by February 11, 2011 at the fol-

lowing address:

Alexandria, MN 56308

If no student, parent or guardian

responds to this public notice, the

school district will assume consent to

destroy the entire record specific to

the student beginning February 14,

2011.

*The district may maintain a per-

manent record, without time limitation,

of a student's name, address and

phone number, his/her grades, atten-

dance record, classes attended, grade

level completed, and year completed.
