Knute Nelson to participate in hospice initiative
Knute Nelson was recently selected to participate in the Targeting Resources Use Effectively (TRUE) project to increase appropriate hospice referrals, improve access and increase length of stay.
The initiative is supported by the federal Centers for Medicare and Medicaid Services (CMS) and is led by Stratis Health, Minnesota’s Medicare quality improvement organization.
As part of the TRUE initiative, Stratis Health is convening community-based organizations to come together as teams for learning, sharing and networking. These teams will have access to improvement experts and evidence-based practices to address the issues of late hospice referrals, short length of stays and barriers to access.
“We continually strive to find ways of enhancing the quality of our care for the betterment of those we serve,” said Angie Urman, senior vice president of operations. “This TRUE initiative is a great example of how we can engage thought leaders and apply best practices in our hospice program to achieve our mission of enriching the lives of everyone we serve.”
