The initiative is supported by the federal Centers for Medicare and Medicaid Services (CMS) and is led by Stratis Health, Minnesota’s Medicare quality improvement organization.

As part of the TRUE initiative, Stratis Health is convening community-based organizations to come together as teams for learning, sharing and networking. These teams will have access to improvement experts and evidence-based practices to address the issues of late hospice referrals, short length of stays and barriers to access.

“We continually strive to find ways of enhancing the quality of our care for the betterment of those we serve,” said Angie Urman, senior vice president of operations. “This TRUE initiative is a great example of how we can engage thought leaders and apply best practices in our hospice program to achieve our mission of enriching the lives of everyone we serve.”

Stratis Health is a nonprofit organization founded in 1971 that leads collaboration and innovation in health care quality and safety. It serves as a trusted expert in facilitating improvement for people and communities.

Knute Nelson, a 501(c)(3) non-profit, faith-based corporation, offers senior housing and health care with a full family of services to the West Central Minnesota region.
