Blandin Foundation awards grant to Knute Nelson Foundation
The Blandin Foundation awarded the Knute Nelson Foundation a $50,000 grant to introduce GrandCare technology to palliative care and hospice patients throughout a 25-county west central Minnesota region where Knute Nelson services.
GrandCare is an all-in-one, in-home monitoring and sensor technology that enhances direct patient care to promote aging in place.
Using a small, touch-screen monitor placed in a patient's home, both patients and their families are able to access resources on topics such as health and wellness, bereavement and chronic disease education.
GrandCare allows patients, caregivers and nursing professionals the ability to access essential patient information to monitor and ensure proactive health care interventions.
For example, medication reminders prompt the patient to take their medication at a scheduled time. Should the dose be missed, a call is placed to follow-up with the patient and/or their caregiver to notify them of the missed dose.
GrandCare also allows patients to utilize social networking components like Skype, Pandora, Facebook, games and brain exercises among other features to more easily communicate with remotely located family and friends.
